Las aftas o llagas que aparecen en la lengua se definen como lesiones ulcerosas de forma redondeada u ovalada, con apariencia blanquecina, que pueden presentar diferentes tamaños. Las heridas bucales, para que reciban el nombre de aftas, deben ser superficiales: sin supuración o cualquier otro signo de infección.

De hecho, las aftas en la lengua, labios, paladar… no son contagiosas, pero sí muy molestas e incluso dolorosas. Lo habitual es que se produzcan de forma más frecuente en niños, preadolescentes, adolescentes y adultos jóvenes, reduciéndose su incidencia conforme aumenta la edad.

Causas de las aftas

No se conoce bien cuáles son las causas por las que aparecen las aftas en la lengua, pero sí que hay algunos factores que pueden actuar como desencadenantes:

  • Morderse la lengua accidentalmente.
  • Falta de sueño.
  • Estrés.
  • Acidez gástrica con reflujo.
  • Tabaquismo.
  • Consumo de bebidas ácidas.
  • Alteraciones hormonales en la mujer.
  • Deficiencia de ciertas vitaminas (C, B12), ácido fólico o minerales como hierro y zinc
  • Consumo de algunos fármacos.

Las aftas de la lengua suelen durar más tiempo que aquallas que aparecen en otras zonas de la cavidad oral, ya que están en contacto continuo con los dientes o los alimentos y sufren frecuentes roces, lo que equivale a pequeños y repetidos traumatismos. En general tardan en desaparecer entre una o dos semanas, dependiendo del tamaño y las agresiones (rozaduras, bebidas ácidas, etc.) que puedan sufrir. Cuanto mayor es su tamaño, más tardarán en desaparecer.

¿Qué riesgos tienen las aftas en la boca?

En este artículo vamos a explicar los tipos de prótesis dentales más habituales que se suelen hacer en la boca de los pacientes.

En primer lugar explicaremos qué es una prótesis dental.

La prótesis dental es un elemento externo que colocamos en la boca del paciente para reparar y/o reforzar las piezas deterioradas, y también para sustituir los dientes que falten.

La primera gran clasificación de prótesis es la que se dividirían en fijas o removibles.

Esto quiere decir que las fijas no se las puede quitar el paciente y las removibles las que el paciente se las puede quitar y poner.

PRÓTESIS FIJAS

Las prótesis fijas son prótesis que se colocan sobre estructuras que están ancladas a los maxilares y sirven de apoyo y sujeción para dichas prótesis.

Estas estructuras sueles ser o dientes o implantes.

PRÓTESIS FIJA SOBRE DIENTES

Aquí encontramos varios tipo de prótesis.

CORONA

Las coronas son prótesis que se colocan sobre el diente y que normalmente lo recubren casi totalmente con el objetivo de reforzar un diente destruido o endodonciado y por lo tanto debilitado.

Hace la función de “casco” de protección.

INCRUSTACIÓN

La incrustación es un elemento que se usa para proteger un diente en condiciones similares a los que se pueden colocar coronas.

Cuando usamos uno u otro

La diferencia fundamental es la menor agresividad de la incrustación, es decir, la corona necesita de una gran eliminación de tejido dentario para su colocación, mientras la incrustación es más conservadora.

Por ello, la incrustación es el tratamiento de elección en dientes que no están endodonciados.

La corona se usa más en dientes endodonciados, pero también las incrustaciones cuando no queramos eliminar mucho tejido dentario.

PUENTES

Cuando a un paciente le faltan uno o varios dientes, pero tiene dientes a ambos lados de la zona pérdida, se puede realizar un puente. Este puente usa esos dientes adyacentes para apoyar toda la estructura.

La principal desventaja de este tratamiento es que hay que hacer una reducción dentaria de esos dientes para que se puedan realizar los apoyos, en consecuencia, en ocasiones provoca sensibilidad. Realmente estamos eliminando el esmalte del diente con lo que el diente sufre un deterioro.

PRÓTESIS FIJA SOBRE IMPLANTES

Este tipo de prótesis son las que se colocan en boca usando como apoyo un implante o varios implantes.  Los implantes son unos elementos de titanio que van roscados al hueso y sustituirían a la que era anteriormente la raíz del diente. Y sobre éste se coloca el diente a sustituir. Generalmente roscado o cementado.

En este caso también se pueden hacer de manera unitaria, es decir, para sustituir un solo diente o en forma de puente para sustituir varios dientes, o toda una arcada de dientes.

¿SE PUEDEN HACER PUENTES CON PILAR MIXTO, ES DECIR, PILAR DENTARIO Y PILAR DE IMPLANTE?

No es aconsejable, pues el diente habitualmente tiene un micromovimiento que le permite el ligamento periodontal y el implante está totalmente soldado al hueso, por lo tanto el micromovimiento del diente puede hacer fracasar el implante.

 

Aunque eso no impide que en casos excepcionales se pueda hacer.

Las protesis fijas sobre dientes o implantes son mayoritariamente de cerámica, con varias opciones, metal-cerámica, Zirconio, Disilicato de litio etc.

En el caso de implantes también se hacen de manera habitual prótesis híbridas, que son prótesis fijas atornilladas con una barra metálica interna que le da resistencia y la parte externa es resina.

Se usa en algunos casos para realzar la pérdida de soporte labial en maxilares atróficos. Es decir cuando se ve el labio hundido.

La principal desventaja de éstas es la fractura de dientes de resina y la retención de suciedad bajo la prótesis.

PRÓTESIS REMOVIBLES

Las prótesis removibles como hemos dicho antes son las que el paciente se las puede quitar.

La subdivisión  que tiene es la siguiente, prótesis parciales o completas.

PRÓTESIS PARCIALES REMOVIBLES.

Son aquellas que sustituyen los dientes faltantes, quedando alguno de los dientes en boca.

Los dos tipos más habituales son los esqueléticos y las prótesis parciales de resina.

Los esqueléticos son prótesis removibles que tienen un armazón metálico y sobre el mismo se colocan los dientes a sustituir. Este armazón tiene unos ganchos y unos topes para ajustar a los dientes y estabilizar el aparato. El armazón metálico al ser fino y pulido es cómodo.

Las prótesis parciales de resina son muy similares a los esqueléticos con la diferencia es que todo el armazón es de resina y también lleva ganchos metálicos y en ocasiones algunos topes. Suelen ser más frágiles e incómodas que los esqueléticos.

Las prótesis completas removibles, son las conocidas como dentaduras y ocupan todos los dientes de la boca y el apoyo lo realizan directamente en encía.

Suelen ser incómodas por el gran paladar de resina que llevan en su parte superior, por la movilidad de la prótesis inferior y en ocasiones la aparición de llagas y roces.

SOBREDENTADURAS

Éstas serían las prótesis completas removibles, que estaría sujetas por implantes pero con un sistema de sujeción que permite el retiro de la prótesis.  Esta sujeción puede venir dada por conexiones macho hembra, barras, caballetes etc.

Consiguen una buena retención y reducción casi total de la movilidad y la posibilidad de que el paciente se la pueda quitar y limpiar.

Eliminan los principales inconvenientes de las dentaduras y son prótesis muy higiénicas y cómodas.

Antiguamente también se usaban como pilares los propios dientes del paciente. Con la aparición de los implantes esta técnica está totalmente en desuso.

CONCLUSIONES

En resumen, gracias a los implantes hoy día hemos aumentado las posibilidades de tratamiento ante la falta de dientes, y dar opciones más cómodas a los pacientes, así como, mantener las posibilidades tradicionales en situaciones que  por determinadas razones no se puedan usar implantes.

Implants | Immediate Loading

Immediate loading is an implant technique that allows a temporary crown to be placed on the dental implant on the same day that the patient undergoes implant surgery to replace one or more missing teeth. Comparison between Invisalign® and Brackets

Time: the duration of the treatment depends on the complexity of the case, normally an orthodontic treatment lasts around a year and a half, two years; but there are shorter Invisalign® treatments according to the needs of each patient, Lite and Express.

Visits to the Orthodontist: visits with braces treatment will have to be once a month, while with Invisalign® visits can be more spaced depending on the number of splints delivered. Likewise, the possible emergencies are less with Invisalign® than with brackets since these can come unstuck and it would be necessary to re-cement it.

The different appliances are tools and it is the orthodontist's responsibility to know and master the different techniques with a solid base of orthodontic knowledge, biology and biomechanics. There are good and bad treatments with Brackets and Invisalign® or any other appliance. The one who does the treatment is the Orthodontist.
